When making a disclosure request by the person himself/herself, please print out the following Application Form A (for the person himself/herself), fill in the required items, have the person himself/herself sign and seal it, enclose Document B for identity verification, and mail it to our "Personal Information Inquiry Desk" mentioned above in 3. If making a request through a representative, please use Application Form A (for the representative).

Please note that application documents must be those specified by our company, and we cannot accept applications in any other format.

Please note that we will not return any documents (including identity verification documents) submitted when requesting disclosure. After responding to your request for disclosure, we will dispose of them in an appropriate manner without delay.
